Needs domains — high-level life domains in plain words. The circle, integrated; pick any, in any order.

A caring conversation — only after they choose to enter. Gentle, private, never a blunt label.

Walk away with a Care Guide — exit any time; we hand over a tailored guidance document to keep.

What it produces

The end of the flow isn’t an answer — it’s a Care Guide: a personalized document built from what they shared and who’s in their household. It lays out options and pathways with phone numbers, links and QR codes, sorted by what to consider today, this week, and eventually. Works on their phone and as a printout.

What it deliberately does not do

No deep, branching intake — it stays short and surfaces directions, not a full assessment.
No eligibility determination — the guide points to what to check; people review details & decide.
No enrollment or sign-ups — the person follows up themselves, or with a navigator, when ready.
