A first draft for prioritizing AHC-HRSN questions
Use this prototype to discuss what each question can reveal, the
response it could unlock, and what the group would change before
implementation. Exact instrument wording and source attribution are
retained below.
The draft assumes staff can identify the affected benefit and timing
from the case record, notice, or a brief prompt. These questions
identify needs; they do not establish whether an HR1 requirement
applies, verify eligibility, or determine an exemption.
Draft group 1 — Detect immediate harm
Field the complete ten-question CMS core, then ask whether the person wants help. Prioritize food, housing, utilities, transportation, and safety responses that can act now. Possible readiness check: A benefits-status and deadline prompt, service directory, consent step, referral owner, and urgent-response protocols are ready.
Why it helps here These are the most direct signals that a SNAP reduction or loss is already changing what the household can eat or afford.
Response to design Offer current food and basic-needs options; record whether the person wants help and whether the connection reaches contact, enrollment, and food received.
Watch before fielding The twelve-month lookback may capture need that predates HR1. Pair the response with timing and benefit-status context rather than attributing the need automatically.

Why it helps here Lost food or medical support can force tradeoffs with rent, utilities, and safe shelter. Unstable housing also makes notices and proof harder to receive and retain.
Response to design Offer housing-stability or shelter navigation and establish a reliable way to receive time-sensitive notices when the client wants assistance.
Watch before fielding A housing response needs separate urgency and safety triage; the screen alone does not rank imminent eviction, homelessness, or unsafe conditions.

Watch before fielding A general transportation referral is weak unless it fits the route, hours, mobility needs, and date that prompted the question.

Watch before fielding Do not treat safety disclosure as an HR1 data point or ordinary referral. Privacy, safe contact, and trained response take precedence.

Response to design Ask permission for each next step and record declined, information only, referral accepted, and follow-up requested as distinct outcomes.
Watch before fielding This is a proposed local addition, not part of the CMS AHC-HRSN instrument. Declining help must not affect benefits or other services.

Draft group 2 — Protect continuity
Add financial strain, mental health and stress, disability, and support questions to identify consequences that can compound a benefit loss or make navigation harder. Possible readiness check: Behavioral-health response, accessibility support, and a way to track contact and service receipt are available.
Why it helps here This question can reveal the broader tradeoffs created or intensified by lost food or health coverage, including rent, care, heating, and other basics.
Response to design Offer benefits assistance and basic-needs navigation based on the expense the client identifies; avoid assuming one generic financial referral fits.
Watch before fielding The source defines no positive threshold. Use the response to begin a conversation, not to assign a risk category automatically.
AHC-11 Financial Strain choose one

Why it helps here Coverage uncertainty, interrupted treatment, food insecurity, and administrative burden can worsen distress. These questions help identify continuity and clinical follow-up needs.
Response to design Use established behavioral-health and crisis pathways, protect existing treatment where possible, and track whether care continued through the benefit transition.
Watch before fielding PHQ-2 is a brief signal, not a diagnosis, and does not ask about suicidal ideation. A defined escalation pathway must exist before fielding it.

Add employment, education or training, child care, and carefully governed substance-use questions when they can open a real next step. Possible readiness check: Programs have confirmed eligibility boundaries, capacity, privacy rules, handoff methods, and follow-up ownership.
Why it helps here For clients who want it, employment help may support income stability or connect them with an approved activity pathway.
Response to design Offer Workforce Development, SNAP E&T, or another suitable pathway only after confirming the client's goal and the program's current fit.
Watch before fielding This question asks whether help is wanted. Do not turn a voluntary needs screen into work-requirement monitoring or assume employment is the right response for every client.

Watch before fielding Responses may create specially protected substance-use records depending on who asks and where they are stored. Resolve governance before implementation.

The next questions relate to your experience with alcohol, cigarettes, and other drugs. Some of the substances are prescribed by a doctor (like pain medications), but only count those if you have taken them for reasons or in doses other than prescribed. One question is about illicit or illegal drug use, but we only ask in order to identify community services that may be available to help you.

Add physical-activity questions after the direct HR1 response is stable and the result can connect to a useful health or community resource. Possible readiness check: The earlier releases are working reliably and this additional information will change the help offered.
Why it helps here These questions add useful health context but are less directly connected to stabilizing a SNAP or Medicaid transition than the earlier groups.
Response to design Use them when a meaningful clinical or community-health response exists and the result will change the support offered.
Watch before fielding Age-specific interpretation is required. Do not let this lower-priority module delay the direct benefit-loss response.
